Acceptance of Terms
By accessing or using Hoss Benefits services, including the Hoss Card and associated platform, you agree to be bound by these Terms of Service. If you do not agree, do not use our services. These terms apply to all members, providers, and enterprise partners.
Description of Services
Hoss Benefits provides a healthcare benefits coordination platform that connects members with supplemental benefit services including food, transportation, pharmacy, labs, home support, DME, caregiving, housing assistance, and meal delivery. We facilitate coordination between health plans, members, and service providers.
Hoss Card Usage
The Hoss Card is a benefits access card linked to your health plan's supplemental benefit allocations. It may only be used for eligible purchases and services as defined by your plan. Misuse, including purchases outside approved categories, may result in card suspension and benefit forfeiture.
Member Responsibilities
You are responsible for maintaining accurate personal information, using benefits only for approved purposes, safeguarding your Hoss Card and account credentials, and promptly reporting any unauthorized use or suspected fraud.
Provider Network
Services are delivered through the Hoss Network of vetted providers. While we credential and monitor all network providers, Hoss Benefits is a coordination platform and does not directly provide medical care, transportation, food, or other services.
Eligibility and Coverage
Benefit eligibility is determined by your health plan. Hoss Benefits verifies eligibility in real time but does not make coverage decisions. Disputes regarding coverage should be directed to your health plan. Benefit allocations and limits are set by your plan and may change.
Intellectual Property
All content, features, and functionality of the Hoss Benefits platform — including text, graphics, logos, and software — are owned by Hoss Benefits and protected by intellectual property laws. You may not reproduce, distribute, or create derivative works without written permission.
Limitation of Liability
Hoss Benefits is not liable for service delays, provider actions, health plan coverage decisions, or indirect damages arising from platform use. Our total liability is limited to the amount of benefits transacted through your account in the preceding 12 months.
Termination
We may suspend or terminate your access if you violate these terms, misuse benefits, or engage in fraudulent activity. You may terminate your account at any time by contacting support. Termination does not affect your underlying health plan benefits.
Governing Law
These terms are governed by the laws of the State of Delaware. Disputes will be resolved through binding arbitration in accordance with the American Arbitration Association rules, except where prohibited by law.
